Thank you for your interest in Collaborative Cannabis Care, a practice limited to Medical Marijuana certification in Pennsylvania. In order to serve you better, either send us your medical records in advance, or bring them to your first appointment.

Pennsylvania has determined that 17 specific conditions qualify for Medical Marijuana certification.  You must have proof that you have one of these conditions.

We will be working collaboratively with your Primary Care Physician and any medical specialists you decide need to know about your Medicinal Marijuana certification.  Please bring names and addresses of any physician who you would like to be notified.

In addition to Medical Marijuana certification, we will be providing dosing recommendations for those who are interested.
